Two forms are required in order to participate in programs at Ortona, as issued by Alberta Gymnastics Federation.
- Notification and Assumption of Risk Form
- Personal Information Protection Act Form
Ortona has shifted to an online waiver system called Privit where your forms can be signed digitally. Forms must be re-signed annually (July 1st to June 30th). To create an account/update your waivers on Privit, please click HERE.
- Please note that all athletes must have both waiver forms signed in order to participate in gymnastics.
- All guardians/parents who are on the floor during any program/party/field trip, must have their own forms signed as well.
